Types of Infarction:
Transmural: atheroma occluding major coronary artery.
Subendocardial: small area in the subendocardial wall of left ventricle.
Silent infarctions = 25%
Prognosis:
50% die within 30 days of attack.
50% of these deaths occur before medical assistance arrives.
30% all deaths occur within the first hour(due to acute arrhythmia)
Prognosis correlates with degree of myocardial necrosis estimated by:
-Rise in serum troponin T.
-Extent of ECG changes.
-Degree of left ventricular dysfunction on echo.
